Requirements to Participate in Track and Field
- A MSHSAA physical - physicals are valid for TWO years from the date issued. We recommend you make a copy of the physical/annual documentation to keep for yourself.
- Annual Documentation Form - this must be completed EVERY YEAR. This form is meant to give medical information filled in by the guardian and provide medical insurance (state requirement) for the school in case of an emergency.
- Impact Testing - Student Athletes must have Impact Testing completed prior to the Track season. Please complete THIS IMPACT TESTING FORM prior to your student completing the Impact Test at school. Impact testing is valid for TWO years from the date tested.
- If your student is in need of Impact Testing, please click here: Impact Testing Sign up to reserve a time slot on 2/6 and 2/15 from 2:15-3:15. If we do not have someone reserve a time slot, we will assume that no one has need of that time frame and that session will be cancelled. If neither of those dates work for you, please email [email protected] to setup a separate time.
- Participation Fee - A one-time, per school year, participation fee of $25. If you have already paid as part of a fall/winter sport, then you do not need to pay again. Make checks payable to the Lakeview Middle School and note “Participation Fee” on the memo line.
- Signed Athlete Contract - This contract outlines the expectations to participate in the Track and Field Team, as well as outlines the reasons that a student may not be allowed to participate in meets or may be terminated from the team. Please complete this with your student athlete so that you are both aware of the expectations.
Turn in all requirements to the office by March 1st. Your runner cannot attend practice (starting Feb. 27) until a MSHSAA Sports Physical Form, Annual Documentation Form, and Impact Testing are on file.
Do I need to do IMPACT Testing or get a physical?

- An upcoming 7th Graders
- A new student to the district
- A student who did not participate in sports last school year
- A student who did not participate in Fall or Winter Sports this year.
Am I eligible to participate in track?
Students must:
- Be 7th graders 14 and under OR 8th graders 15 and under.
- Have no more than 1 F during the previous grading period (2nd quarter 2022-2023).
- Have good conduct both in and out of school.
- Have no more than one failing grade and above a 2.0 GPA during the season. (3rd quarter 2022-2023)
- Have participated in 14 practices to compete at meets. If a runner misses practices, they are not eligible for the 1st meet!
- Be at school at least a half day on meet days to participate in meets.
